The first one is from February 15, 1989 which ironically also features that one promo of The Weather Channel (the one with Dan Chandler).

Now we have video evidence that the switch from all-capitals to mixed-case listings occurred at some point in early 1994.

I think thats when the black list software went de-commissioned and was replaced with the blue grid.

Some new finds on Prevue, including a Prevue Latino clip from 1995 on YouTube.
Prevue Latino (from an unknown cable provider in Latin America) in October 1995 by "Telearchivos Retro":

(Another example of program "changes" in some channels due to censorship and at the 23:14 mark, they've used the new station ID of Prevue Latino with the "eye" logo, which was used by the U.S. Prevue in '96, depsite their header at the grid carrying the old logo.)
