I do not have cable or DBS any more, but apparently Cox Communications in Tulsa has this now (Passport?). We did have TV Guide Interactive until 2002, and I vaguely remember Prevue Interactive with TCI Digital Cable circa 1998...

At about 3m22s into the video (Part 1), someone opens the menu and then selects Diagnostic Mode, where we see the select code XA22659.

This was due to a user they were trying to permaban who kept uploading preview videos of version 3 of the emulator to YouTube:
